Purpose:
  From February 2012 we started robotic cardiac surgery using da Vinci surgical system. This study investigated the feasibility and sort term result of robotic cardiac surgery.
Materials and Methods:
  More than 400 patients operated on for cardiac disease since 2012 in National Taiwan university hospital. For coronary artery bypass surgery, all of them received robotic assisted internal mammary artery harvesting, and followed by tiny thoracotomy direct coronary artery anastomosis or total endoscopic coronary artery bypass.
  For mitral valve or tricuspid valve disease patients, all of them received peripheral cannulation for cardiopulmonary bypass. The exposure were all made from left atrium and assisted by da Vinci left atrial retractor. Repair was performed in all degenerative mitral valve disease, and replacement was performed in rheumatic mitral disease. If the patient had atrial fibrillation, left atrial appendage closure and left atrium ablation was performed.
Results:
  There is no conversion to sternotomy in all robotic cardiac surgeries. The internal mammary arteries are all taken down with good flow quality.
  Repair of the mitral valve was feasible in all degenerative disease, infective endocarditis and functional regurgitation patients. These repair involved annuloplasty in 90% patients, valve resection in 57%, artificial chordae implantation in 90%. Mitral valve replacements were performed in 113 patients with rheumatic heart disease. There was no operation mortality. One-year followed, one patient who was diagnosed to have functional mitral regurgitation dilated cardiomyopathy underwent heart transplantation, the others were doing well in functional class I.
Conclusion:
  Robotic cardiac surgery is feasible by using standardized robotic surgery protocol. Mitral valve repair offers good immediate results. For coronary artery disease, the robotic system offers good surgical exposure and magnification which made the mammary flow in good quality.
