Development of a coordinated medical-life care geriatric care system

  Aging population has greatly increased the burden of national insurance covered health care system in Taiwan. According to the statistics of national health insurance in 2017, 13.3% aged 65 or more accounted for 36.8% general medical cost and 33% hospitalization. Multiple co-morbidity and frailty are the main features of medical need in aged population. Current specialist oriented but not primary physician oriented care makes the condition even worse. Lack of coordinated primary physician taking care of the elderly is one of the key problem of medical system facing aging society. The other important issue is the insufficiency of life care system. Non-satisfactory life care support lead to prolonged hospitalization, long waiting list for hospitalization, and emergency department burnout.
  To overcome the current tough condition, the first thing is to establish a coordinated primary care physician system for the elderly is necessary. Person center care focusing on quality of life, frailty prevention and active aging should be introduced to the population and put into practice instead of disease oriented care. The second is to build up a life care supporting system for those who are partial or complete disable and in need. Owing to the trend of fewer offspring in the family, caring the aged parents or grand-parents become a huge burden for the most family. A tailor made life care supporting system should be established based on the Taiwanese’s need and affordability. Shortage of well-trained manpower in life care system is a critical condition. Therefore, the key element in the establishment of life care system is to train adequately and pay properly professionals. The third is to introduce the active aging to the population. The elderly will live in an active life style with the age –friendly environment and view the death as a life event, but not a medical event.
